Consent Form for the Use of Personal Data

Parents and/or guardians of the student are aware that the Medical Release Form (Atestado Médico), with the respective report, must be submitted to the School, by the beginning of classes, in cases where the student has any health condition that result in restriction of physical activities or any other academic/educational activity. Failure to submit the Medical Release Form (Atestado Médico) makes the student considered able to participate in all school activities, including Physical Education classes as directed by the teacher. At any time, after the enrollment/re-enrollment date, in the event that the student is affected by a condition that, due to the medical context, makes it impossible or reduces the condition of being fit for regular activities, the parent/guardian must immediately communicate to the school, and submit the medical report.

In order to offer greater security for the school community respecting the best interest and broad protection of children and adolescents, we are following strict protocols implemented as a result of the pandemic moment: a facial detection system that will allow individualized identification of students to enter school.
